Forms

Overview

The DOLU.ai Forms module lets you build web forms for lead capture, data collection, surveys, and event registrations — without writing any code. Every form submission automatically flows into your CRM, creating or updating contacts, triggering Salesbot workflows, and notifying your team in real time. Forms are a critical bridge between your website visitors and your sales pipeline.

You can embed forms on your website, share them as standalone links, or trigger them inside the DOLU.ai Web Widget during a chat conversation. Combined with the Visitors module (which tracks who is on your site) and the Salesbot module (which automates follow-up), Forms become the starting point for fully automated lead qualification and nurturing.

How to Access

Dashboard → Left Menu → Forms

You can also access form submissions from:

  • A contact's detail page → Forms tab (shows all forms submitted by that contact)
  • The Notifications panel (real-time alerts for new form submissions)

Key Features

Drag-and-Drop Form Builder

  • Field types: Text, email, phone, number, date, dropdown, checkbox, radio buttons, file upload, textarea, hidden fields
  • Conditional logic: Show or hide fields based on previous answers (e.g., show "Company Name" only if the user selects "Business" as account type)
  • Multi-step forms: Break long forms into multiple pages with a progress bar
  • Field validation: Required fields, email format validation, phone format, minimum/maximum length, custom regex patterns
  • Pre-fill fields: Use URL parameters to pre-fill fields (useful for campaign tracking)

Form Design and Branding

  • Custom styling: Change colors, fonts, button text, background, and border radius to match your brand
  • Logo upload: Add your company logo to the form header
  • Thank you page: Customize the success message or redirect to a custom URL after submission
  • Responsive design: All forms are automatically mobile-friendly

CRM Integration

  • Auto-create contacts: Each submission can automatically create a new contact in the CRM with the form data mapped to contact fields
  • Auto-update existing contacts: If the email or phone matches an existing contact, update their record instead of creating a duplicate
  • Custom field mapping: Map form fields to any CRM contact field, including custom fields
  • Tag assignment: Automatically tag new contacts based on which form they submitted (e.g., "webinar-signup", "free-trial-request")
  • Pipeline placement: Automatically create a deal and place it in a specific pipeline stage upon form submission

Submission Management

  • Submission list: View all submissions in a searchable, filterable table
  • Submission details: Click any submission to see the full response with timestamps
  • Export: Download submissions as CSV or Excel for external analysis
  • Spam protection: Built-in Cloudflare Turnstile verification to prevent bot submissions

Step-by-Step Usage

Creating a Lead Capture Form

  1. Go to Dashboard → Left Menu → Forms.
  2. Click "+ New Form".
  3. Enter a Form Name (e.g., "Contact Us", "Free Trial Request", "Quote Request").
  4. Use the drag-and-drop builder to add fields:
    • Drag Name (text field) from the field palette
    • Drag Email (email field)
    • Drag Phone (phone field)
    • Drag Message (textarea)
    • Drag Service Interest (dropdown with your service options)
  5. Mark required fields by toggling the Required switch on each field.
  6. Configure Conditional Logic if needed (e.g., show a "Budget" field only if they select "Enterprise" in the dropdown).
  7. Click Save.

Setting Up CRM Mapping

  1. In the form editor, go to the Settings tab.
  2. Under CRM Integration, enable Auto-Create Contact.
  3. Map each form field to a CRM field:
    • Form "Name" → Contact "Full Name"
    • Form "Email" → Contact "Email"
    • Form "Phone" → Contact "Phone"
    • Form "Message" → Contact "Notes"
    • Form "Service Interest" → Contact custom field "Interested Service"
  4. Set the Auto-Tag to "website-lead".
  5. Optionally enable Create Deal and select the pipeline and stage (e.g., "Sales Pipeline → New Lead").
  6. Click Save.

Publishing and Embedding

  1. In the form editor, go to the Share tab.
  2. Choose your distribution method:
    • Direct link: Copy the unique URL to share via email, social media, or messaging
    • Embed code: Copy the HTML/iframe snippet and paste it into your website
    • Popup/Modal: Get a JavaScript snippet that triggers the form as a popup on button click
  3. Enable Turnstile Verification to prevent spam bots.
  4. Click Publish. The form is now live.

Setting Up Automated Follow-Up

  1. Go to Dashboard → Left Menu → Salesbot.
  2. Create a new workflow with the trigger: "Form Submitted" → [Your Form Name].
  3. Add follow-up actions:
    • Send Email: Auto-send a thank-you email with relevant information
    • Send WhatsApp: Send a personalized WhatsApp message to the new lead
    • Assign Agent: Automatically assign the lead to a sales rep based on round-robin or territory rules
    • Wait + Follow-Up: Wait 24 hours, then send a follow-up message if no response
  4. Activate the workflow.

Use Cases

E-Commerce — Product Inquiry Forms

An online store places a "Request a Quote" form on product pages. When a customer fills it out, a contact is created, tagged with "quote-request", a deal is created in the pipeline, and the sales team receives an instant notification. The Salesbot sends an automated WhatsApp message within 5 minutes.

Healthcare — Patient Registration

A clinic embeds a patient intake form on their website. Fields include personal info, insurance details, medical history checkboxes, and preferred appointment type. Submissions create a contact in the CRM and trigger an appointment booking flow.

Education — Course Enrollment

A training institute uses multi-step forms: Step 1 collects personal info, Step 2 asks about course preferences, Step 3 handles billing information. Conditional logic shows different course options based on the selected category. Enrollment data feeds directly into the CRM for student management.

Real Estate — Property Viewing Requests

Each property listing page has an embedded form. The form captures the visitor's name, phone, and preferred viewing times. A deal is automatically created linked to the property account, and the assigned agent gets an instant in-app notification.

Tips & Best Practices

Keep Forms Short

Every additional field reduces your conversion rate. Only ask for information you truly need at this stage. You can collect more details later through conversations or follow-up forms.

Use Hidden Fields for Tracking

Add hidden fields with URL parameters to track which campaign, ad, or page generated the lead. For example, add a hidden "utm_source" field that auto-fills from the URL. This data appears on the contact record for attribution reporting.

Enable Turnstile Verification

Always enable Cloudflare Turnstile on public forms to prevent spam submissions. DOLU.ai uses a fail-open pattern, meaning forms still work even if the verification service is temporarily unavailable.

Test Before Publishing

Use the Preview button in the form editor to test the form on desktop and mobile. Submit a test entry and verify that the CRM mapping, tags, and automation triggers all work correctly.

Troubleshooting

Form submissions not creating contacts

  • Check that Auto-Create Contact is enabled in the form's Settings tab.
  • Verify that the email or phone field is mapped correctly — at least one identifier is needed to create a contact.
  • Look at the submission list to confirm the submission was received (it may be a mapping issue rather than a submission issue).

Embed code not displaying the form

  • Ensure the embed code is pasted inside the <body> of your HTML page, not the <head>.
  • Check for conflicts with your website's Content Security Policy (CSP). You may need to whitelist the DOLU.ai domain.
  • Verify that JavaScript is enabled in the browser.

Spam submissions getting through

  • Enable Turnstile Verification on the form.
  • Add a honeypot field (a hidden field that only bots fill in) — submissions with this field filled are automatically discarded.
  • Check if the spam submissions are coming from the same IP range and consider rate-limiting.

Conditional logic not working

  • Ensure the triggering field is placed above the conditional field in the form layout.
  • Double-check the condition values match exactly (including case sensitivity for text fields).
  • Test in Preview mode with different input combinations.

FAQ

Q: How many forms can I create? A: The number of forms depends on your subscription plan. Check Settings → Billing for your current limits.

Q: Can I duplicate an existing form? A: Yes. From the forms list, click the three-dot menu on any form and select Duplicate. This creates a copy with all fields, settings, and mappings intact.

Q: Can form submissions trigger Salesbot workflows? A: Yes. In the Salesbot module, use the trigger type "Form Submitted" and select the specific form. You can build any automation chain from there.

Q: Do forms support file uploads? A: Yes. Add a File Upload field to your form. Uploaded files are attached to the contact record in the CRM. You can restrict file types and set maximum file size in the field settings.

Q: Can I view form analytics? A: Yes. Each form has an Analytics tab showing total views, submissions, and conversion rate over time. You can also see this data aggregated in the Reports module.

Q: Are form submissions stored if the CRM mapping fails? A: Yes. All form submissions are stored in the submission list regardless of whether the CRM mapping succeeds. You can manually process any failed mappings from the submission detail view.
